The evaluation form provides the opportunity to assess, with the entire team, how the intervention unfolded after the report. To fill out an evaluation form, navigate to the details screen of a report and click on the "Add Evaluation Form" button (you may need to scroll down): report overview -> report -> evaluation form button (button with a '+' sign).
In addition to the survey created by the administrator, it is possible to add photos. An administrator can then view and generate results in the dashboard as a comprehensive report for the alert.
